Payscout Application Checklist

PAYSCOUT APPLICATION CHECKLIST

  • Application – 2 Signatures on page 1, 2 Signatures on page 3, all vital information filled out (ss #, DL number, Owner/officer info, etc…)
  • Copy of App signors Drivers License – (If there is more than 1 owner listed on the app, we also need a copy of their DL)
  • Copy of SS-4 Form/W-9 form – (this must be a legal tax document from the IRS on their letterhead to the company stating the company’s Fed ID number)
  • Copy of voided check(s), or bank letter
  • Last 3 months bank statements – for checking account(s)
  • 2011 and 2012 company financials/tax returns along with 2013 YTD.
  • Articles of Incorporation
  • If this is an existing merchant who has already been processing, we will need last 3 months processing statements
  • Collection Policy Acknowledgement Form filled out and signed
