NAM-style form – This page demonstrates the NAM-style payment form template. Page has a contact us page with room for address information, and the check/credit card pages are selectable from the payment form itself (not the landing page). Convenience fee implemented.
Yakima-style form – This is a simple landing page linking to a simple payment form. The end user must check the box on the landing page indicating agreement before being allowed to proceed. No convenience fee.
Liberty-style form – This page has expanded landing page text, a contact us page, a checkbox to indicate agreement, and a 5% convenience fee implemented on the form.
Solace-style form – This page has a drop-down account selection menu for illustration purposes. The listed account displays a payment form. The other account displays a message stating “Please call in to make a payment.” No convenience fee.
Very Customized – Responsive (PBC – Phone Browser Compatible)
Very Customized Form – This page has a highly customized background, shows how well we are able to match a user’s existing website. Must agree to terms before being able to move forward. Ability to select between check and credit cards at the top of the form.

Collector Initiated Demo
Collector Initiated Form Demo (copy and paste this link in because it redirects to the apsmemberservices login) http://www.apsmemberservices.com/apswebforms/client/demo/collector – This page has a self filling “system user” section that pulls in the username after they login to the member services area. It has 2 drop downs and an agree to terms check box that all must be filled out in order to move on.
